Health Requirements

Physical Exam 

The Illinois School Code, Chapter 122-Section 27-8, requires that all students have a Physical Exam completed on the state mandated Illinois form by a physician licensed to practice medicine in all its branches at the following times:
  1. Students entering an Illinois school for the first time.  (Physical must be documented on the required State of Illinois form).
  2. Students entering Kindergarten, sixth, and ninth grades.  All students must present documented evidence of adequate immunizations for school attendance as required by the State of Illinois. More information on required immunizations.
All students attending Kindergarten must show Proof of Lead Screening. Please make sure lead results are written in the space provided on the physical form. 

Vision Exams

Vision exams are required by the State of Illinois for students entering Kindergarten and new students entering an Illinois school for the first time. These exams need to be on the required State of Illinois Vision Exam form and turned in by October 15th of the current year. 

Dental Exams

Dental exams are required for students in Kindergarten, sixth and ninth grades. These exams need to be on the required State of Illinois Dental Form and turned in by the start of school.
 
Students are not allowed to attend school if the PHYSICAL and IMMUNIZATION  requirements are not met by OCTOBER 15th as mandated by the State of Illinois.
